Zaburzenia rozwojowe układu stomatognatycznego stanowią istotny problem kliniczny, szczególnie u dzieci i młodzieży, gdyż mogą wpływać na funkcję narządu żucia, rozwój twarzoczaszki, estetykę oraz jakość życia pacjenta. Ich etiologia związana jest z oddziaływaniem różnorodnych czynników działających na organizm w okresie prenatalnym, okołoporodowym oraz po urodzeniu. W zależności od momentu wystąpienia czynnika uszkadzającego wyróżnia się wady wrodzone oraz nabyte.
Uzębienie mleczne odgrywa kluczową rolę w prawidłowym rozwoju układu stomatognatycznego, uczestnicząc w procesach żucia i mowy, utrzymaniu miejsca dla zębów stałych oraz kształtowaniu szczęki i żuchwy. Okres płodowy, noworodkowy i niemowlęcy jest szczególnie istotny dla rozwoju struktur jamy ustnej, a fizjologiczne czynności, takie jak ssanie, połykanie i oddychanie, mają znaczący wpływ na kształtowanie prawidłowych relacji zwarciowych oraz rozwój mięśniowy. Do najczęściej występujących wrodzonych zaburzeń rozwojowych uzębienia należą hipodoncja, oligodoncja oraz anodoncja, różniące się liczbą brakujących zawiązków zębowych. Hipodoncja charakteryzuje się brakiem od jednego do pięciu zębów i może występować jako wada izolowana lub w przebiegu zespołów genetycznych. Oligodoncja obejmuje brak sześciu lub większej liczby zębów i często współistnieje z innymi nieprawidłowościami rozwojowymi. Najcięższą postacią agenezji jest anodoncja, polegająca na całkowitym braku uzębienia. Wady te mogą prowadzić do zaburzeń funkcji żucia, fonacji, estetyki twarzy oraz nieprawidłowego rozwoju struktur kostnych. Leczenie protetyczne dzieci z zaburzeniami rozwojowymi uzębienia jest procesem długotrwałym i wieloetapowym, wymagającym współpracy zespołu specjalistów, w tym stomatologa, ortodonty, protetyka oraz logopedy. W okresie rozwojowym podstawową rolę odgrywają uzupełnienia ruchome, które umożliwiają odbudowę funkcji żucia, poprawę estetyki oraz stymulację wzrostu wyrostków zębodołowych. Po zakończeniu wzrostu twarzoczaszki możliwe jest zastosowanie bardziej zaawansowanych metod leczenia, takich jak uzupełnienia stałe czy implanty zębowe. 41 Prawidłowo zaplanowana rehabilitacja protetyczna układu stomatognatycznego u dzieci z hipodoncją, oligodoncją i anodoncją pozwala nie tylko na poprawę funkcji oraz estetyki jamy ustnej, lecz także na wsparcie prawidłowego rozwoju psychofizycznego pacjenta oraz zapobieganie pogłębianiu się wad zgryzu i deformacji twarzoczaszki.
Developmental disorders of the stomatognathic system constitute a significant clinical problem, particularly in children and adolescents, as they may affect masticatory function, craniofacial development, facial aesthetics, and the overall quality of life of patients. Their etiology is associated with the influence of various factors acting on the organism during the prenatal, perinatal, and postnatal periods. Depending on the timing of the damaging factor, congenital and acquired defects can be distinguished.
Primary dentition plays a crucial role in the proper development of the stomatognathic system, as it participates in mastication and speech, maintains space for permanent teeth, and contributes to the formation of the maxilla and mandible. The fetal, neonatal, and infancy periods are particularly important for the development of oral structures, and physiological functions such as sucking, swallowing, and breathing have a significant impact on the establishment of proper occlusal relationships and muscular development. The most common congenital developmental disorders of dentition include hypodontia, oligodontia, and anodontia, which differ in the number of missing tooth germs. Hypodontia is characterized by the absence of one to five teeth and may occur as an isolated defect or as part of genetic syndromes. Oligodontia involves the absence of six or more teeth and is often associated with other developmental abnormalities. Anodontia represents the most severe form of dental agenesis and is defined by the complete absence of teeth. These conditions may lead to impairments in mastication, phonation, facial aesthetics, and abnormal development of skeletal structures. Prosthetic treatment of children with developmental dental disorders is a long-term and multistage process that requires cooperation among a multidisciplinary team, including a dentist, orthodontist, prosthodontist, and speech therapist. During the developmental period, removable prosthetic appliances play a fundamental role, as they enable 42 the restoration of masticatory function, improvement of aesthetics, and stimulation of alveolar ridge growth. After completion of craniofacial growth, more advanced treatment options, such as fixed prosthetic restorations or dental implants, may be considered. Properly planned prosthetic rehabilitation of the stomatognathic system in children with hypodontia, oligodontia, and anodontia allows not only for the improvement of oral function and aesthetics but also supports normal psychophysical development and helps prevent the progression of malocclusions and craniofacial deformities. |
